Possible fix for #653, division by zero is never good and any baudrate not matching standard ones can cause this. If custom baudrate is supported fallback to this even if cardmhz == mhz.

Added support for AD-Teknik USB Phoenix as well (using protocol = mp35). This device can now be clocked to 3.57, 3.68 or 6.00MHz.
Also updated the init sequence slightly, could possibly solve some problems with not detected AD-Teknik reader.
